£45,780 – £51,393 p.a. Full time, 37.5 hrs p.w.

The North London Hospice has been providing palliative and end of life care for the communities of North London since 1984. We have an 18 bed in-patient unit providing hospice care for those with life limiting illnesses. There are community teams of specialist nurses, doctors, consultants, social workers and allied professionals supporting individuals at home, in the 3 North London Boroughs of Barnet Enfield and Haringey. We operate a health and wellbeing centre, outpatient clinics, a hospice at home service and bereavement support. All this is achieved by the commitment, passion and expertise of the hospice staff and volunteers. Our aim is to provide the very best of palliative and end of life care for the residents of Barnet, Enfield and Haringey.

We are looking for Band 7 community CNS’s to join a team covering the boroughs of Enfield and Barnet; the teams are based in Winchmore Hill and Finchley. The team is multi-professional and is supported by a hospice-at-home service.

We support our patients and their loved ones to enable them to be cared for in their preferred place of care providing specialist advice to patients professionals, patients and their families. You will need to be committed to providing compassionate, dignified care, have excellent communication skills and variety of experience. Candidates should have experience of specialist palliative care and supporting people with life limiting conditions which should include a knowledge of symptom control and caring for patients at the end of their lives.

Candidates will work as part of multidisciplinary team managing a shared caseload ensuring patients are seen by the right person at the right time. Successful candidates will be expected to support the rapid response and duty nurse roles and continue to work with community partners as part of the integrated model of care.
